About Us
The Command Surgeon is the senior medical advisor to the U.S. Army Central Commander, as well as the command's senior staff for all health service support policy, plans, engagements, and exercises.
The Surgeon Directorate oversees U.S. military medical operations across the USARCENT area of responsibility. We sustain the health of our forces, support ongoing operations, and strengthen partner-nation military medicine capacity.
Deployment Readiness
All U.S. uniformed Service Members, U.S. Department of Defense (DoD) Civilian employees, contractors, and volunteers deploying to CENTCOM’s area of responsibility must meet U.S. Central Command (CENTCOM) physical and psychological standards as listed in USCENTCOM MOD 17:

Medical Waivers
Medical deployment standards for deploying personnel may be waived under certain conditions. The USARCENT Surgeon is the medical waiver authority for Army Personnel deploying to the CENTCOM AOR (except for SOCCENT Soldiers). Follow the steps in the Medical Waiver Process section below to submit a request.
Waiver Process
Service Members, Civilians or Contractors requesting a Medical/Behavioral Health waiver must complete the Medical Waiver Request Form linked below. This can be completed by the deploying personnel or a medical provider.
MEDICAL WAIVER REQUEST FORM[JTS2]
Scan and submit this form via email to usarmy.shaw.usarcent.mbx.surg-waiver@army.mil from your .mil email address. In order for the USARCENT Surgeon to make an informed decision, please include the following with your waiver request:
· Full name and Last four of the SSN
· Status (AD/AR/ARNG for military; DAC or Contractor for civilians)
· Installation of assignment, major command, or contract name
· Rank (does not apply to contractors)
· MOS/AOC or duty description for Civilian/Contractor
· Pertinent medical history (including a detailed history of the condition causing the non-deployable status, any co-morbid conditions, current medications, pertinent test results, and recommendations from the medical provider)
If you have been seen at a VA Facility or have VA health records that may need to be reviewed by the USARCENT Surgeon Waiver Team please complete a Release of Information (DD Form 2870) so a determination on your waiver request can be made in a timely manner. Submit the DD 2870 with your waiver submission once completed.
